Registration

The following types of questions will be asked in the application:

• Household information (address and phone numbers)

• Parent information (work and cell phone numbers, email addresses)

• Student information (demographic and health/medication information)

• Emergency Contact (addresses and phone numbers)
   

In addition, applicants new to Baldwin County Schools or returning after having withdrawn will need to upload the following documents to the application:

• Birth Certificate (certified copy)

• Social Security Card

• Immunization Record (Georgia form 3231, current/up-to-date)

• Ear, Eye, and Dental screening (Georgia form 3300)

• Current Proof of Residence (lease or mortgage statement/property tax records in parent's/legal guardian's name)

• Current Utility Bill (power, water, gas, cable in parent's/legal guardian's name)

• Parent/ Guardian Picture ID 

If the utility bill/lease is in someone's name other than the parent/legal guardian, the following documentation must be presented in addition to the above documents:

• Notarized affidavit from the person whose name is on the POR/utility bill stating that the parent/legal guardian and child live at the address indicated on the POR/utility bill.This affidavit can be picked up at the school your child is zoned for. This letter must be notarized by a licensed notary public in order to be accepted.

  Note: Required fields are marked with a red asterisk, and the district will receive the data exactly as it is entered. Please be careful of spelling, capitalization and punctuation. Dates should be entered as MM/DD/YYYY and phone numbers as xxx-xxx-xxxx.

Students that have relocated and are requesting a school transfer will need to present 2 new proofs of address to the transferring school.  

• Current Proof of Residence (lease or mortgage statement/property tax records in parent's/legal guardian's name)

• Current Utility Bill (power, water, gas, cable in parent's/legal guardian's name)
